I will use the purchased laser component in a safe and responsible way and will incorporate said laser component into a finished product meeting all laser safety standards, and will use said laser component for a legal purpose.

I am legally and solely responsible for the use of purchased laser components, their improper use, and their use in any finished product.

I am legally responsible for any injury to myself or to any other person resulting from the use, assembly, or operation of the purchased laser component and their use in any finished product.

I am purchasing a laser component to be used as a component to be integrated in a system of my own design, and am legally responsible for any and all liabilities accruing therefrom.

I fully understand that any laser component purchased from Advanced Photonic Sciences is sold as a component to be integrated into a customer’s end product, and is therefore exempt from compliance with the appropriate requirements of FDA 21CFR, Sections 1040.10 and 1040.11, for complete products.
